Menu Bar→Lists→Custom Lists and Field Names→Employees→

Name of Custom Field #1: Next of Kin
Name of Custom Field #2: Next of Kin Phone Number
Name of Custom Field #3: Next of Kin Relationship

CLICK OK, now look at an employee Card File, and fill in relevant information.
